Passengers may wish to complete a Questionnaire available on the Visit Japan website before departure from Australia for a smooth and quick arrival procedure. Alternatively, you will need to complete this upon arrival into Japan at your first point of entry. This does not need to be completed if entering Japan by Cruise port.

How Can I Complete the Form?

Your flight and final itinerary will assist with certain sections of the form including entry/exit points and accommodation details.
